2008 National Forum for Implementers of School-Wide PBS
It is not necessary to chose your breakout sessions at the time of registration. At the end of the opening plenary, teams will have 10-15 minutes to develop a plan on what information they will be seeking and decide which team members will go to which sessions.
For your convenience, we have both an on-line registration form and the option to download the published registration brochure and mail/fax a paper registration form. To register, please chose one of the options below.
Your registration will be confirmed via email within 24 hours if registering online or 2 weeks if registering via fax or mail. If you do not receive a confirmation within these timelines, please call (708) 482-4860 to verify that we have received your registration.

Registration Fee:
There is a $180.00 per person registration fee to attend this forum. Registration fee includes continental breakfast for both days of the forum and lunch on October 30th. All fees are due on or before the first day of the conference.
Payment Options:
Unfortunately, we are unable to accept credit card payments. Checks/money orders must be in US dollars and made payable to SASED-IL PBIS Network. Please send all payments to the address below to ensure your payment is properly credited. We cannot accept incorrectly made out checks.
Important Note:
All checks incorrectly made out or not drawn on US funds will be returned.
Purchase Orders:
Purchase orders will be accepted as a commitment to pay registration fees. All Purchase Orders must include the name and date of the forum and the names of all registrants. Please indicate your type of payment and check/money order/Purchase Order number on your registration form.

Cancellation Policy:

Breakout sessions will be divided into three concurrent strands by themes:
SWPBS Basics
Specific Skill Fluency Building
Systems Implementation
Each block of breakout sessions will include two SWPBS Basics sessions, three Specific Skill Fluency Building sessions, and three Systems Implementaiton sessions.
